Prevalence of Intestinal Metaplasia, Dysplasia and Helicobacter pylori Infection by Aging

Abstract
BACKGROUND
: The recent studies shows that Helicobacter pylori (H. pylori) related chronic gastritis leads to dysplasia, intestinal metaplasia and gastric cancer. The aim of this study was to determine the association between precancerous lesion of stomach (dysplasia, intestinal metaplasia) and H.pylori infection in the elderly.
METHODS
: 469 patients were enrolled this study, Patients with intestinal metaplasia or dysplasia (case, n=148) were compared with controls (without intestinal metaplasia and dysplasia, n=321) about H. pylori positive rates H. pylori positivity was confirmed by histologic determination of endoscopic biopsy specimens.
Results
: 1) Among study populations H. pylori positive rates were 51.8% (55.8% in male, 40.7% in female). It was significantly higher in male (p=0.001). 2) There were no significant differences in sex distribution between cases and controls, but in old age, the cases were more frequent than in young age (p<0.001). 3) Age distribution of H. pylori positive rates were not significant between cases and controls. 47.5% in intestinal metaplasia, 43.4% in dysplasia, 21.3% in dysplasia+intestinal metaplasia were H. pylori positive. H. pylori positive rates were more higher in controls than cases, followed by in decreasing order by the intestinal metaplasia group, the dysplasia group, and those having both (p=0.003). 4) Age and sex adjusted H. pylori positive rates were more higher in controls (42.7%) than in cases (25.0%)(p<0.009).
CONCLUSION
: The prevalence of intestinal metaplasia and dysplasia was age dependent. But we couldn't explain the carcinogenic effect of H. pylori, because H. pylori positive rates were lower in cases than in controls

초 록

배경 :

연령증가에 따른 위점막의 장형화생, 이형성의 빈도와 이러한 병변에서의 H. pylori의 감염률을 조사하여 노년층의 위암과 H. pylori와 관련성을 간접적으로 알아보기 위해 본 연구를 시행하였다.

방법 :

1997년 9월부터 1998년 5월말까지 9개월 동안 대전시 일개 대학병원 종합검진센터와 가정의학과 외래를 방문하여 위내시경하 조직 검사를 받았던 469명을 연구대상으로 하였다. 조직 소견상 장형화생과 이형성 등을 보였던 집단을 환자군(148명)d로 이러한 소견을 보이지 않은 집단을 대조군(321명)으로 하여 H. pylori 감염빈도를 조사하였다.

결과 :

1) 연구 대상자의 H. pylori 양성률은 51.8%(남자 55.8%, 여자 40.7%)로 남자에서 H. pylori 감염빈도가 유의하게 높았다(p=0.001).

2) 환자군과 대조군에서 성별분포는 유의한 차이가 없었으나, 연령이 증가하면서 유의하게 환자군 비율이 증가하였다(p<0.001).

3) H. pylori 양성률은 환자군과 대조군 모두에서 연령에 따른 차이는 유의하지 안았고((p>0.05), 대조군에서 55.1%, 장형화생에서 47.5%, 이형성에서 43.4%, 장형화생+이형성에서 21.3%의 순으로 H. pylori 양성률이 유의하게 낮아졌다(o=0.003).

4) 나이와 성별을 보정한 H. pylori 양성률은 환자군에서 25.0%, 대조군에서 42.7%로 대조군에서 의의 있게 높았다(p=0.005).

결론 :

여령 증가에 따라 장형화생과 이형성의 빈도가 높아졌다. H. pylori 양성률은 대조군보다 환자군에서 낮았기 때문에 위 전암병변의 점막 조직에서 H. pylori의 유무를 알아보는 것으로 H. pylori의 발암성을 증명할 수 없을 것으로 생각된다.
