Relationship between Homocysteine Level, White Matter Lesion and Cognitive Decline

호모시스테인 농도와 백질변성 및 인지장애와의 관계(예비 연구)
Abstract
BACKGROUND
Hyperhomocysteinemia increase the risk of vascular disease, dementia and cognitive impairment. The effect of homocysteine on cognitive function may be as a consequence of direct neurotoxicity or indirectly through cerebrovascular disease. Thus we examined the association between homocysteine levels and cognitive function, and assess whether this may be mediated by cerebral white matter lesions.
METHODS
Non-dementic participants were recruited from March to December 2005, and underwent brain MRI. White matter hyperintensities(WMH) were rated visually by 4-class WMH grades. Informations of age, gender, education and vascular risk factor were obtained from interview. Cognitive function were assessed by using K-MMSE and HDS-R. The cross-sectional relationship between homocysteine level and cognitive test score was assessed by multiple regression analysis.
RESULTS
Severe WMH groups more exposed to vascular risk factor such as age, hypertension and higher levels of homocyteine. More severe WMH grades also had lower K-MMSE and HDS-R score. Higher group of homocysteine concentration were significantly related to poorer performances on K-MMSE and HDS-R tests. In multiple linear regression analyses for the relationship between homocysteine and cognitive performances, higher homocysteine level was also related to lower cognitive performances. Adding cerebral WMH changes to the level of homocysteine did not significantly change the effect of homocysteine on cognitive ability, and cerebral WMH had a separate and additive effect with homocysteine.
CONCLUSIONS
Elevated homocysteine level is associated decreased cognitive score and cerebral white matter lesion on MRI is independent association to the cognitive abilities.

초 록
연구배경: 고호모시스테인혈증은 혈관성 질환 뿐만 아니라, 치매와 인지기능의 위험을 증가시키는 것으로 알려져 있다. 호모시스테인의 인지기능에 대한 영향이 혈관성 질환을 통한 간접적인 영향인지 직접적인 신경독성의 결과인지는 아직 밝혀지지 않았다. 본 연구는 혈중 호모시스테인의 농도와 인지기능 그리고 뇌자기공명영상에서 보이는 뇌백질변성의 관계를 알아보고자 한 예비 연구이다.

방법: 2005년 3월부터 12월까지 춘천성심병원 신경과에 내원하여 뇌자기공명영상을 촬영한 환자들을 대상으로 인지기능 검사와 호모시스테인을 측정하였고, 일반적인 뇌졸중 위험인자를 조사하였다. 12시간 이상 공복 후 호모시스테인을 측정하였고, 환자의 인지기능의 수준을 평가하기 위하여 K-MMSE와 HDS-R을 시행하였으며 백질변성의 정도는 Shelten의 modified version을 이용하여 등급을 나누었다. 호모시스테인과 인지기능 사이에 백질변성의 역할을 분석하기 위하여 다중 회귀분석(multiple linear regression analysis)을 사용하였다.

결과: 전체 80명 환자의 평균 호모시스테인의 농도는 11.61umol/L였으며, 백질변성의 정도에 따라 4군으로 분류하였을 때 고혈압의 유무가 각 군사이에 유의한 차이를 보였으며, 인지기능을 검사한 K-MMSE와 HDS-R 점수와 호모시스테인 농도가 각 군 사이에 유의한 차이를 보였고(p<0.01), 백질변성의 정도가 심한군일수록 인지기능의 점수가 낮아지고, 호모시스테인 농도가 높아지는 양상을 보였다. 호모시스테인 농도에 따른 분석을 위해 혈중 농도에 따라 4개의 그룹으로 나누었을 때 K-MMSE, HDS-R 점수는 호모시스테인 농도에 따른 각각의 그룹간에 유의한 차이가 있었으며, 나이와 성별, 교육정도를 교정한 다중회귀분석모델로 분석했을때 호모시스테인은 인지기능과 연관성을 가지고 있었으며, 뇌백질변성이라는 요소를 추가해도 인지기능에 대한 호모시스테인의 영향 정도는 의미있게 변하지 않았으며, 뇌백질변성의 영향은 독립적인 것으로 분석되었다.

결론: 호모시스테인은 다른 심혈관계 위험인자와 상관없이 인지기능장애의 중요한 위험인자로 보이며, 이런 관계는 뇌자기공명영상에서 보이는 백질변성과 같은 미세혈관병증에 의해 매개되는 것으로 보이지 않는다.
