Relation of Regular Exercise Habit to Bone Mineral Density in Korean Climacteric Women

한국 갱년기 여성의 골다공증 관련 요인과 운동 실천과의 관계
Abstract
BACKGROUND
Even if the effect of regular exercise on the prevention against osteoporosis has been confirmed by many researchers, the research which evaluates the effect of regular exercises by adjusting the differences between climacteric women with respect to the status of menopause, and other health behaviors, has been few. The main purpose of this study was to evaluate the risk of osteoporosis and its relation to regular exercise.
METHODS
We studied 222 climacteric women aged 40~59 years who visited the health care centers run by Chunchon branch of Korean Association of Health for preventive health examination at 2002/3~2002/11. The study subjects were asked to self-administer to the questionnaire including the items for the evaluation of the status of regular exercise and other characteristics of health behaviors and nutritional intakes. Bone mineral density was measured in L1-4 using dual-energy X ray absorption device.
RESULTS
Among the total 222 subjects, 52 women (23%) have been exercising regularly, among whom 22 subjects (42%) were in the phase of menopause. Exercise group had higher levels of intake of protein, calcium, iron, vitamin B1, niacin, and vitamin C than non-exercise group. In multiple logistic regression analysis, aged 50 years (OR: 5.87, 95% CI: 2.26~15.23), menopause (OR: 2.65, 95%CI: 1.03~6.82), body mass index over 25 (OR: 0.24, 95%CI: 0.10~0.59) were significant. There was no significant difference for the bone mineral density between exercise and non-exercise groups. T score seemed to be slightly higher in the regular exercise group (-1.88 vs -1.79) compared with non-exercise group, but the difference was not significant (p>0.05).
CONCLUSION
Age, menopause, low body mass index are risk factor of osteoporosis. Postmenopausal women who do not exercise showed a tendency of decreased bone mass density and increased frequency of osteoporosis. Further study is needed to prove the relation of regular exercise to bone mass density in Korean climacteric women.

초 록

연구배경: 규칙적인 운동이 골다공증의 예방에 효과가 있음이 인정됨에도 불구하고, 폐경 여부나 다른 건강행위의 실천유무를 고려하여 갱년기 여성들에서의 운동에 의한 효과를 비교 평가한 국내 연구는 아주 미미한 실정이다. 이 연구의 목적은 갱년기 여성에서 흔한 골다공증과 연관된 위험요인을 탐색하고, 골밀도에 대한 규칙적인 운동과의 관계를 평가하는 것이다.

방법: 2002년 3월부터 11월까지의 기간 중 춘천시 건강관리협회 부속의원에 건강검진을 목적으로 방문한 40세에서 59세 사이의 갱년기 여성 222명을 대상으로 하였다. 연구 대상자의 규칙적인 운동의 실천 여부의 평가와 다른 건강행위들의 특성은 자기기입식 설문으로 조사 하였다. 골밀도는 이중에너지 방사선 흡수 계측기를 이용하여 첫 번째에서 네 번째 요추에서 측정하였다.

결과: 222명의 전체 대상자 중 52명(23%)이 운동실천을 하였고, 그중 폐경후 여성은 22명(42%)이었다. 운동군에서 단백질, 칼슘, 철분, 비타민 B1, 비타민 C, 니아신 섭취가 비운동군에 비해 유의하게 높았다. 골다공증 관련 요인으로 연령 50세 이상(OR: 5.87, 95% CI: 2.26∼15.23), 폐경 상태(OR: 2.65, 95% CI: 1.03∼6.82), 체질량 지수 25 이상(OR: 0.24, 95%CI: 0.10∼0.59)이 회귀분석에서 유의한 변수로 나타났다. 한편, 규칙적인 운동을 하는 여성은 운동을 하지 않는 군에 비해 높은 골밀도 경향을 보였으나 유의하지는 않았다(T-score: -1.88 vs -1.79, p >0.05; OR: 1.81, 95% CI: 0.78∼4.32)

결론: 연령, 폐경상태, 낮은 체질량지수는 골다공증의 위험 요인이다. 규칙적인 운동과 갱년기 여성의 골밀도와 관계는 연구 대상자 중 운동 실천 대상자를 늘린 후속 연구가 필요하다.
