The Relations hip Between Posts troke Depression and Functional Recovery of Rehabilitation Inpatients

뇌졸중 입원 환자의 우울증과 기능적 회복의 상관관계
Abstract
BACKGROUND
To identify the prevalence and risk factors of poststroke depression(PSD) in patients admitted to department of rehabilitation medicine after stroke, to compare functional recovery of depressed patients and that of non-depressed patients, and to recognize the most useful depression scale that can predict functional recovery. METHOD: Of the hospitalized stroke patients in the department of Rehabilitation Medicine, 24 patients who were communicable were included in this study. To evaluate PSD, Beck depression inventory(BDI) and Korean form of Geriatric depression scale(KGDS) were used as self-rating scales. Hamilton depression scale(HAM-D) was used as an objective scale. Functional Independence measure(FIM) was measured at admission and discharge to evaluate functional recovery.
RESULTS
In the 24 subjects, 17 patients(70.8%) and 9 patients(37.5%) were depressive by BDI and HAM-D. Of the 19 elderly patients, 16(84.2%) were depressive by KGDS. Factors such as age, level of education, religion, etiology or location of stroke were not significantly associated with PSD. And FIMscores were not significantly different in the depressed patients and non-depressed patients. The correlation coefficients of BDI, KGDS, HAM-D and FIMgain or efficiency were not statistically significant.
CONCLUSION
The prevalence of PSDwas high in our study, but no association was found between PSD and functional recovery.

초 록

연구배경: 뇌졸중 후 우울증은 비교적 흔한 합병증으로 알려져 있으나, 이의 유병률, 위험인자, 기능적 회복 정도와의 상관관계에 대해서는 보고자 및 우울증 평가도구 등에 따라 많은 차이를 보여왔다. 이에 저자들은 뇌졸중으로 입원한 환자를 대상으로 우울증의 유병률과 위험인자를 조사하고 우울증의 유무에 따라 재활치료 후 기능적 회복 정도에 차이가 있는지 비교하였으며, 기능적 회복정도를 가장 잘 예측할 수 있는 우울증 평가 척도를 알아 보고자 하였다.

방법: 뇌졸중 후 재활의학과에 입원한 환자들 중 의사소통이 가능한 24명의 환자를 대상으로 하여 다양한 우울증 평가도구를 이용하여 뇌졸중 후 우울증에 대한 평가를 하였고, 입·퇴원 당시의 Functional Independence Measure(FIM) 척도를 비교하여 기능적 회복 정도를 평가하였다.

결과: 뇌졸중 후 우울증의 유병률은 37.5∼84.2%로 자가 보고식 주관적 평가도구와 임상가에 의한 객관적 평가도구에 따른 많은 차이를 보였고, 성별, 연령, 종교 유무, 교육 정도, 뇌졸중의 원인 및 위치에 따라 뇌졸중 후 우울증 정도의 의미 있는 차이는 없었다. 또한 각 평가도구에 따라 우울증이 있는 환자군과 없는 환자군으로 분류하였을 때 뇌졸중 후 우울증과 기능적 회복 정도에는 유의한 상관관계가 나타나지 않았다.

결론: 뇌졸중 후 우울증의 발생과 관련한 연구는 우울증에 대한 평가 및 기능적 회복과의 상관관계를 알아보는데 의의가 있다고 하였을 때, 본 연구의 결과는 뇌졸중 후 우울증의 정확한 평가에 대한 정보를 제공할 수 있었다고 생각한다. 그러나 이번 연구에서 나타난 여러 제한점으로 인해 뇌졸중 후 우울증 발생에 대한 위험인자 및 기능적 회복과의 상관관계 등에 대해서 단정적 결론을 내리기는 어려우며, 이에 대해서는 추후 더 많은 수의 환자를 대상으로 한 보완 연구가 필요할 것으로 생각한다.
