Changes of Cytokines in CSF of Patients with Alzheime r's Disease and Vascular Dementia

알쯔하이머병과 혈관성치매 환자의 뇌척수액에서 사이토카인의 변화
Abstract
BACKGROUND
Alzheimer's disease(AD) and vascular dementia(VaD) are common types of dementia. Despite the heterogeneous nature of both AD and VaD, inflammation may also play an important role in the pathogenesis of these diseases. However, to our knowledge, there is little evidence reported on differences of cytokines. The aim of this study was to analyze the patterns of CSF cytokine release in patients with AD and VaD, to reveal the difference of inflammation processes between AD and VaD, and to relate the levels of these cytokines to the severity of the dementias.
METHODS
We selected two groups of elderly with neurocognitive and functional impairment described as ADand VaD, and measured CSF levels of cytokines. Obtained data were analysed with respect to neuropsychological tests(Korean version of Mini-mental State Examination: K-MMSE, Clinical Dementia Rating: CDR, Global Deterioration Scale: GDS) and clinical parameters. We measured the CSF levels of IL-1 , TNF- and TGF- in 25 patients with AD, 22 patients with VaDand 9 normal controls. Cytokines were measured by sandwich ELISA(enzyme linked immunosorbent assay) method.
RESULTS
Compared with controls, patients with AD and VaD showed higher CSF levels of TNF- (p=0.046, p=0.060) and TGF- (p=0.038, p=0.040), but not in IL-1 . VaD patients showed more impairment in CDR and GDS than patients with AD. The levels of IL-1 , TNF- and TGF- in CSF did not differ significantly between patients with AD and VaD. Neuropsychological tests and clinical parameters were not correlated with IL-1 , TGF- and TNF- levels in CSF.
CONCLUSION
These findings show that cytokines in CSF are elevated in patients with AD and VaD. These results suggest that these cytokines might not only play a role in the pathogenesis of AD but also in the pathogenesis of VaD. Although the initial event triggering the neurodegenerative process in ADversus VaDmay be different, it may reveal a similar cascade of cytokine in response to neuronal injury.

초 록

연구배경: 알쯔하이머병과 혈관성치매의 원인과 계속 진행하는 기전에 대해서는 아직 잘 알려져 있지 않다. 병인론에 있어 염증이 관계한다는 것은 여러 가지 방법으로 증명되었다. 특히, 사이토카인은 염증반응에 있어 중요한 역할을 하는 단백질이다. 알쯔하이머병과 혈관성치매에서 사이토카인을 직접 비교한 문헌은 거의 없고, 그 관련된 몇 편 논문에서의 결과들도 다양하다. 본 연구의 목적은 알쯔하이머병과 혈관성치매 환자의 뇌척수액에서 사이토카인을 정량적으로 측정하여 분석하고, 두 질환군에서 사이토카인의 차이로 인한 염증반응의 차이를 비교하며, 임상적 변수와 이들 사이토카인간의 상관관계가 있는지를 알아보고자 하였다.

방법: 환자군은 알쯔하이머병과 혈관성치매의 두 군으로 각각 NINCDS-ADRDA와 NINDS-AIREN 기준을 만족하는 25명과 22명이었고, 대조군은 환자군과 나이, 성별, 교육수준 등에서 차이가 없는 정상인 9명으로 하였다. 신경심리 검사로 K-MMSE, Clinical Dementia Rating(CDR), Global Deterioration Scale(GDS) 등을 측정하였다. 검체는 뇌척수액을 채취하여 사용하기 전까지 -70℃에 냉동 보관하였다가, IL-1 , TNF- , TGF- 농도를 Sandwich ELISA 방법을 이용하여 정량적 분석을 실시하였다.

결과: 뇌척수액 IL-1 은 알쯔하이머병군, 혈관성치매군, 대조군에서 유의한 차이가 없었다. 뇌척수액 TNF- 의 기하평균은 알쯔하이머병군, 혈관성치매군, 대조군에서 각각 91.84 pg/mL, 52.28 pg/mL, 7.82 pg/mL이며, 알쯔하이머병군과 대조군의 비교에서는 유의하였고(p=0.046), 혈관성치매군과 대조군의 평균값 비교에서는 유의하려는 경향을 보였다(p=0.060). 뇌척수액 TGF- 의 기하평균은 알쯔하이머병군 166.88 pg/mL, 혈관성치매군 120.30 pg/mL, 대조군 12.24 pg/mL로, 알쯔하이머병군과 대조군의 비교에서 유의한 차이가 있었고(p=0.038), 혈관성치매군과 대조군의 평균값 비교에서도 유의한 차이가 있었다(p=0.040) 뇌척수액 TNF- 와 TGF- 는 두 환자군의 상호 비교에서는 유의한 차이가 없었다. 유병기간, K-MMSE, CDR 및 GDS 등의 임상적 변수와 혈청 및 뇌척수액의 IL-1 , TNF- , TGF- 등의 사이토카인과 상관관계는 이들 모두에서 유의하지 않았다.

결론: 알쯔하이머병과 혈관성치매에서 염증 및 면역 반응에 관여하는 사이토카인이 비슷하게 증가되어 있어, 두 질환에서 사이토카인 연쇄 반응이 비슷하게 일어남을 확인할 수 있었다. 사이토카인 중 전염증반응과 항염증반응의 역할을 하는 각각의 사이토카인이 모두 증가되어 있어, 두 질환의 병인론에 있어 이들 사이토카인들이 중요한 역할을 수행함을 추정해 볼 수 있다.
