The Effect of Spacer on the Bronchodilator Response in the First Medical Examination of Old Age

초진 노인환자에서 흡입보조기구의 사용이 기관지 확장제 반응도에 미치는 영향
Abstract
BACKGROUND
S: When measuring lung function and response to bronchodilator, MDI(metered-dose inhaler) is commonly used but unfamiliarity of its use and cold sensation by the puffed gas decrease reliability of the result. Spacer can reduce the cold freon effect and undesired oropharyngeal deposition caused by the rapid evaporation of the propellant and there are many studies which showed more effectiveness of spacer on the treatment of children with asthma but no study whether it is effective on the bronchodilator response test in the first medical examination of old age. Therefore, we tested whether the use of spacer can reduce the cold freon effect and improve the bronchodilator response in the first medical examination of old age.
METHODS
Two hundred of elderly patients( 65years) who had never used MDI were measured the bronchodilator response. Subjects were randomised to either spacer-user or spacer-nonuser. Twenty minutes after 400 g fenoterol was administered, FEV1 (forced expiratory volume in one second) was measured. Bronchoconstriction was defined as a decrease in FEV1 by 10% or greater after bronchodilator inhalation. We further devided each group into normal or obstructive group, obstructive group was defined as FEV1<80% of predicted and FEV1/FVC<70%.
RESULTS
In normal group, spacer-user(n=83) showed greater bronchodilator response than spacer-nonuser(n=66), 6.43% vs 3.81% respectively(p<0.05) and two case of bronchoconstriction occured only in spacer-nonuser. In obstructive group, there is no significant difference in bronchodilator response between spacer-user(n=18) and spacer-nonuser(n=33), 12.32% vs 11.16% respectively(p>0.05) but brochoconstriction(n=1) occured only in spacer-nonuser.
CONCLUSION
Spacer improved bronchodilator response and prevented bronchoconstriction, in the first medical examination of old age.
초 록

연구배경: 일반적으로 기관지 확장제 반응 검사를 할 때 정량식 흡입기가 가장 보편적으로 사용되지만, 부정확한 사용법과 흡입시 증발에 의한 냉감이 결과에 영향을 주며, 오히려 기관지수축 반응을 일으킬 수도 있다. 이런 단점을 보완하기 위해 흡입보조기구의 사용이 치료의 효용성을 향상시킬 수 있다고 연구되었지만, 진단에 있어서 흡입보조기구의 사용이 기관지 확장제 반응에 미치는 영향은 연구되지 않아 호흡기영역에서 가역적 기도폐쇄를 증명하기 위해 일반적으로 가장 먼저 시행하는 기관지 확장제 반응검사를 시행함에 있어서 초진 노인환자의 경우 흡입보조기구의 사용이 기관지 확장제 반응 결과에 큰 영향을 나타낼 것으로 생각되어 초진 노인 환자를 대상으로 하여 흡입보조기구의 사용이 기관지 확장제에 대한 반응과 기관지수축 반응에 영향이 있는지에 대하여 연구해 보았다.

방법: 200명의 초진 노인 환자(65세 이상)를 대상으로 무작위로 흡입보조기구를 사용한 군과 흡입보조 기구를 사용하지 않은 군으로 나눠 기관지 확장제 반응검사를 시행했으며, 폐기능에 따라 정상군과 폐쇄성군으로 나누어 비교 분석하였다. 기관지 확장제 반응검사에서 FEV1이 10% 이상 감소한 경우를 기관지수축 반응으로 정의하였다.

결과: 정상군에서는 흡입보조기구를 사용한 군(83명)이 흡입보조기구를 사용하지 않은 군(66명)에 비해 기관지 확장제 반응이 유의하게 증가하였고(6.43% vs 3.81%, p<0.05), 흡입보조기구를 사용하지 않은 군에서만 2명의 환자에서 기관지수축 반응이 나타났다. 폐쇄성군에서는 흡입보조기구를 사용한 군(18명)이 흡입보조기구를 사용하지 않은 군(33명)에 비해 기관지 확장제 반응이 증가하였지만, 통계학적으로 유의한 차이가 없었고 기관지수축 반응은 흡입보조기구를 사용하지 않은 군에서만 1명의 환자에서 나타났다.

결론: 초진 노인환자에서 흡입보조기구의 사용은 기관지 확장제 반응을 향상시켰으며, 기관지수축 반응을 감소시켰다.
