Effects of demographic characteristics and sensory function on the scores of Korean version of Mini-Mental State Examination (MMSE-K) of elderly persons living in a home for the Aged

양로원 노인의 인구학적 특성과 시력, 청력이 한국판 인지기능검사(MMSE-K)에 미치는 영향
Abstract
BACKGROUND
The MMSE-K has been used widely to screen cognitive impairment of the elderly in Korea. However, the MMSE-K was validated only for elderly in the 60th and 70th, and did not consider the effect of vision and hearing impairment. So, this study was conducted to investigate the effect of demographic characteristics and sensory function on the scores of MMSE-K of elderly persons living in a home for the aged, who had low educational attainment, and high proportion of old age.
METHODS
The MMSE-K was tested for 163 elderly persons living in a home for the aged in Seoul, by the interviewer who had been taught about the test methods. Vision and hearing were measured by the near vision card (40 cm), and 'whisper test' respectively RESULTS: Of the 162 subjects, 21 (13%) were excluded due to aphasia, severe vision and hearing impairment, and poor communication. The 141 study subjects (68 male, 73 female) had a mean age of 78.7 years (+/-7.1 years), and a mean MMSE-K score of 19.1 (+/-6.1). The mean score of women, elderly greater than 80 years, elderly with no education, and elderly with vision impairment was less than that of men (p<0.05), elderly below 80 years, elderly with 1 or more years of education (p<0.05), and elderly with no vision impairment (p<0.05), respectively. However, the mean score of elderly with hearing difficulty was not less than that of elderly with normal hearing (p>0.05). In the multiple linear regression analysis with MMSE-K scores as the dependent variable, age (p<0.05), years of education (p<0.05) and vision (p<0.05) were significantly associated with MMSE-K scores, but sex and hearing impairment did not show significant relationship (p>0.05).
CONCLUSION
These findings represent the cross-sectional relationship between MMSE-K performance and education, age, and vision among the institutionalized elderly. So, in using MMSE-K in the institutionalized elderly, we should take these variables into account. And the development of new adjustment methods of the MMSE-K will be needed.

초 록

배경 : 현재까지 국내에서 노인들의 인지기능을 선별하는 도구로서 한국판 간이정신상태검사(MMSE-K)가 많이 쓰여 왔다. 하지만 이 검사는 주로 60-70대 노인에서 그 타당도를 입증한 것이며, 시력과 청력의 영향을 고려하지 않았다. 따라서 학력수준이 낮고 고령 노인이 많은 양로원 노인에서 인구학적 변수, 시력, 청력 등이 MMSE-K 검사에 어떠한 영향을 미치는지 알아보고자 본 연구를 시행하였다.

방법 : 서울 시내의 일개 양로원에 거주하는 163명의 노인을 대상으로 조사방법에 대해 사전 교육을 받은 조사자들이 직접 면담을 통하여 MMSE-K를 시행하였다. 시력은 40cm용 근거리 시력표를, 청력은 'Whisper test'를 이용하여 측정하였다. MMSE-K교정점수 21점 미만을 인지기능 장애, 21점 이상을 정상으로 삼았으며, MMSE-K 점수와 여러 변수들과의 관련성을 student t-test, 다중회귀분석 등을 이용하여 비교 분석하였다.

결과 : 의사소통이 불가능하거나 검사에 응하지 않은 22명을 제외한 141명(87%)을 분석대상으로 삼았으며, 이중 여자가 52%, 평균 연령은 78.7세(±7.1세)였으며, 청력 및 시력 장애의 유병률은 각각 28%, 43%였으며, 인지기능 장애는 57%였다. MMSE-K의 평균 교정점수는 19.1점(±6.1)이었다. MMSE-K의 평균 교정점수가 유의하게 낮게 나온 변수들로는 여자(p<0.05), 80세이상 노인(p<0.05), 무학 노인(p<0.05), 무학 노인 중에서도 시력 장애가 있는 (p<0.05) 등이었으나, 청력 장애는 MMSE-K 점수에 영향을 미치지 않았다(p<0.05). MMSE-K 점수를 종속변수로 하여 다변수 분석을 시행하였을 때 연령(p<0.05), 교육 기간(p<0.05), 시력(p<0.05) 등이 여전히 유의하였고 성별과 청력에 의한 영향은 관찰되지 않았다.

결론 : MMSE-K 검사가 무학 노인에게 일부 항목에 점수를 더해주는 보정 방식을 채택하고 있음에도 불구하고, 양로원 노인들의 교육 수준은 여전히 MMSE-K 점수에 유의한 영향을 미치고 있었다. 또한 연령과 시력에 의한 영향도 무시할 수 없는 수준이므로, 양로원 노인들을 대상으로 MMSE-K 검사를 이용하려면 위의 세 변수를 함께 고려해야 하며, 이를 감안한 새로운 보정 방식의 개발이 필요하다고 사료된다.
